Flu Vaccine Consents

 

The Immunisation Team will be in school Tuesday 4th October to give all pupils their flu vaccine. Please complete this consent form below

If you do not wish your child to be immunised you must still complete the form indicating this choice.

The closing date to give permission is Midnight on Sunday 25th September 2022. Please see the email sent on 16th September.
